General Group:
1.
P2P partner administers informed consent in EPIS to participant
2.
Participant information is entered into EPIS system
3.
EPIS system places participant into general services group
4.
P2P partner enters participant information into Salesforce and selects “General Services” for program placement
5.
P2P partner provides participant with a list of agency services that are available and verbally refers individual to Career Navigator who will recommend assessments and referrals using a business as usual approach
Expanded Group:
1.
P2P partner administers informed consent in EPIS to participant
2.
Participant information is entered into EPIS system
3.
EPIS system places participant into expanded services group
4.
P2P partner enters participant information into Salesforce and selects “Expanded Services” for program placement. Salesforce notifies Career Navigator via email that they have a new client.
5.
P2P partner provides participant with a list of agency services that are available and verbally refers program participant to a Career Navigator for assessments.
6.
Career Navigator meets with client.
a.
Two mandatory assessments Career Navigators must administer:
  • Career Coach
  • BESI
b.
Discuss support services client needs to complete assessments and barriers to program participation
  • All support services should be entered into Salesforce
    • Enter support service referrals to core partners into Salesforce.
    • Referrals for support services to non-core partners should be entered in Salesforce in the case notes field.

Career Navigator uploads mandatory assessment results into Salesforce as attachments and enters results on “Customer Assessments” links

Career Navigator uploads mandatory assessment results into Salesforce as attachments and enters results on “Customer Assessments” links
a.
Use the results of Career Coach Assessment to discuss employment areas of interest and skill
b.
If the participant scores high (between 31-40 on the emotional/physical portion of BESI), partner should administer two additional assessments:
  • Two optional assessments.
    • CAGE AID
    • PHQ9
  • If client answers “Yes” to any question on either of these assessments, Career Navigator should refer participant to Office of Vocational Rehabilitation.

Career Navigator works with client to determine if participant will pursue work, education, or a combination of the two.
10.
Career Navigator completes Individualized Career Plan (ICP) with participant and gets participant to sign based on #9.
a.
Education Path
  • Career navigator should check to see if the education or training the participant is interested in is offered at KCTCS
  • If yes, direct refer to KCTCS Success Coach and adult education
    • Adult Education administers TABE test and/or other appropriate tests and uploads results to Salesforce
    • KCTCS Success Coach discusses education options and work study with participant
  • If the education or training the participant is interested in is not provided by KCTCS, refer to Adult Education for testing and:
    • Look for a university or proprietary school that offers the education or training.
  • Career navigator should work with participant to assess their need for support services.
  • All support services should be entered into Salesforce
    • Enter support service referrals to core partners into Salesforce.
    • Referrals for support services to non-core partners should be entered in Salesforce in the case notes field.
b.
Work Path
  • Participant is assigned an EKCEP Success Coach
  • Participant completes a soft skills training with EKCEP Success Coach
  • EKCEP Success Coach works with Career Navigator to determine work trajectory
  • Work experience
    • Refer to Adult Education for TABE testing and upload to Salesforce as an attachment
    • EKCEP Success coach helps participant with no prior work experience identify employment options
  • On the job training
    • The team helps participant find an on the job training opportunity.
  • Internship
    • The team works with client to find an industry appropriate internship.
  • Direct hire
    • The team works with client to help them find employment with a sustainable wage.
  • Job Advancement
    • The team helps clients advance in their current employment.
  • EKCEP Success Coaches should work with all participants who decide to follow the work path to assess their need for support services needs
  • All support services should be entered into Salesforce
    • Enter support service referrals to core partners into Salesforce.
    • Referrals for support services to non-core partners should be entered in Salesforce in the case notes field.
      • Training
      • Housing
      • Food/Clothing
      • Transportation
      • Job-Related Materials
      • Medicare-Health
      • Food Stamps/ TANF/KTAP
      • Health Insurance
      • Medicaid/Medicare
      • Child/Family Care
      • Pell/KHEAA Grant
      • Other
  • Career Navigator should also Case Note the referral that was made

Work/Education combo path
  • Career Navigator should check to see if the education or training the participant is interested in is offered at KCTCS
  • If yes, direct refer to KCTCS Success Coach and adult education
    • Adult Education administers Adult Basic Education test (TABE) and/or other tests as appropriate and uploads results into Salesforce as an attachment.
    • KCTCS Success Coach discusses education options and work study with participant
  • If the education or training is not provided by KCTCS, refer to Adult Education for testing and:
    • Adult Education administers Adult Basic Education test (TABE) and/or other tests as appropriate and uploads results into Salesforce as an attachment.
    • Look for a university or proprietary school that offers the education or training.
  • Participant is referred to an EKCEP Success Coach

  • EKCEP Success Coach works with Career Navigator and KCTCS Success Coach to determine work trajectory
